Consumers had an opportunity to receive $5.00 by mail with the purchase of two (2) Fresh Express salads. Participants mailed in the proof-of-purchase tab from the DVD, a cash register receipt from the DVD purchase dated April 4, 2006 through June 30, 2006, cash register receipts from the purchase of the salads and a hand printed, completed mail-in certificate which can be downloaded at freshexpress.com. The mail-in rebate was valid between April 4th to June 30, 2006.
